 As Nurse Care Managers, we create personalized care plans and coordinate medical services for individuals with complex or chronic conditions. Our goal is to enhance comfort, ensure safety, and simplify the complexities of medical care, empowering our patients and their support teams to lead healthier and more independent lives.  

 We can fill pill boxes on a weekly, biweekly, or monthly basis. Additionally, we monitor for any adverse reactions and can promptly alert the doctor or family if issues arise or if a medication supply is running low. 

 Doctor's appointments often involve important discussions that can impact a patient's health goals and interventions. Our nurses can attend these appointments, advocate for the patient, and provide the family with a detailed summary and any concerns. Basic transportation can be included in this service if needed. 

Our team is well-versed in wound management. We provide both surgical and non-surgical care for chronic and acute wounds. Skin tears and similar injuries often require immediate attention, and we prioritize urgent needs, frequently visiting within a few hours to help avoid a trip to urgent care. 

We provide individualized newborn support in the comfort of your home. Overnight care may include feeding and burping, diaper changes, soothing and settling, monitoring the newborn throughout the night, preparing bottles or assisting with feeding routines, and helping establish healthy sleep and nighttime patterns.

 Emergency Department visits and hospital stays can be overwhelming. Our nurses, with extensive hospital experience, can help translate complex medical information and assist patients and their families in advocating for their personal health goals. 

 We understand the emotional and physical challenges of living with a terminal illness. Our team is dedicated to supporting both your loved one and your family through a multi-faceted approach. This includes creating a comprehensive care plan centered around the patient's wishes, offering emotional support and facilitating life review, suggesting comfort medications, connecting with external resources when needed, and coordinating care among specialists and the care team.

 We can administer prescribed IV antibiotics, TPN, or IV hydration through a PICC line or Port, all in the comfort of your home. 

 In the days following surgery, we provide post-operative wound care, monitor for infection, unmanaged pain, or any acute complications. We can also communicate these findings with your surgeon and loved ones. 

 Our nurses are trained to provide peace of mind during flights that require basic nursing care. We can travel from Denver International Airport or any regional Colorado airport to destinations worldwide!

We offer personalized post-operative care and convenient pick-up services to support a smooth and stress-free recovery. We can also travel with you to to your procedure location and assist with any logistics and care that are needed. 

Concerned about your blood pressure or oxygen levels? We can come to you to perform a check and determine the next necessary steps. 

 We offer nurse caregiving services for any special event, including weddings and vacations. Our services include administering medications, transfers (to and from a wheelchair or with a walker), feeding, incontinence care, oxygen management, and more.
