Our nurses can attend these appointments, advocate for the patient, and provide the family with a detailed summary and any concerns. Basic transportation can be included in this service if needed.
Emergency Department visits and hospital stays can be overwhelming. Our nurses, with extensive hospital experience, can help translate complex medical information and assist patients and their families in advocating for their personal health goals.
Our team is well-versed in wound management. We provide both surgical and non-surgical care for chronic and acute wounds. Skin tears and similar injuries often require immediate attention, and we prioritize urgent needs, frequently visiting within a few hours to help avoid a trip to urgent care.
We understand the emotional and physical challenges of living with a terminal illness. Our team is dedicated to supporting both your loved one and your family through a multi-faceted approach. This includes creating a comprehensive care plan centered around the patient's wishes, offering emotional support and facilitating life review, suggesting comfort medications, connecting with external resources when needed, and coordinating care among specialists and the care team.
We can administer prescribed IV antibiotics, TPN, or IV hydration through a PICC line or Port, all in the comfort of your home.
In the days following surgery, we provide post-operative wound care, monitor for infection, unmanaged pain, or any acute complications. We can also communicate these findings with your surgeon and loved ones.
